[Bizgres-general] Bizgres update to use postgres 8.2 codebase underway

Updated bitmap index code has been checked in. This checkin is synced with
Gavin's latest patch to pgsql-patches with some cleanups and fixes,
especially the fix to the segfault problem Mark Kirkwood reported earlier.
