[Bizgres-general] Bitmap Index on-disk format documentation

It has been a quiet week here, so I thought I would have a go at 
documenting the on-disk format for the new bitmap indexes:

The section entitled 'Data Structures' is a bit clumsy, (work in 
progress - suggestion welcome). I have also included a simple worked 
example that reads a bitmap index directly. Enjoy.
